Katy ISD requires that each student traveling with the choir have a Medical/Travel Form on file. Paetow High School uses Rank One to track Medical/Travel Forms for all sports and extracurricular activities. This video will help you register a student in Rank One, as well as how to fill out the Medical/Travel forms. Once filled out, these forms can be submitted for any extracurricular activities or sport that supports Rank One.
